Why Am I Waking Up at 3 AM Every Night?: Causes & What Actually Helps
Understanding the Role of Hormones, Stress & Perimenopause
It's 3:07 AM.
Your eyes suddenly open.
You glance at the clock, sigh, and roll over, hoping sleep will return. Instead, your mind starts to wander. You think about tomorrow's to-do list, your children, work, that conversation you had earlier that day. Or perhaps your mind feels completely calm, yet you're simply… awake.
If this sounds familiar, you're not alone.
Many women notice that somewhere in their late thirties or forties, their sleep begins to change. They fall asleep without much trouble, but wake in the early hours of the morning and struggle to drift off again. Others experience this long before perimenopause, often during periods of prolonged stress, after having children, or when life simply feels overwhelming.
It's no surprise that social media is full of explanations. One video tells you it's cortisol. Another blames your liver. Someone else insists it's blood sugar or a magnesium deficiency.
The truth is, our bodies are far more complex than that.
As a women's health coach, one of the things I find most fascinating is how beautifully interconnected our bodies are. Sleep doesn't exist in isolation. Neither do your hormones. Your nervous system, metabolism, reproductive hormones, circadian rhythm and daily habits all communicate with one another. When one system changes, the others often respond too.
So if you've been wondering why you keep waking around 3 AM, let's explore what the research actually tells us.
Is It Normal to Wake During the Night?
Surprisingly, yes.
Waking briefly during the night is a completely normal part of human sleep. Throughout the night, we move through different sleep stages in cycles of roughly 90 minutes. At the end of each cycle, it's common to briefly wake before falling asleep again, often without remembering it the next morning.
The difference is whether you fall back asleep easily. If you're lying awake for long periods, or these awakenings happen night after night and leave you feeling exhausted during the day, it's worth exploring what might be contributing.
According to the latest European Insomnia Guideline, difficulty staying asleep is rarely explained by one single cause. Instead, sleep is influenced by a complex interaction between our biology, psychology, and daily behaviours. These factors often overlap and influence one another, which is why understanding the bigger picture is so important. (Riemann et al., 2023)..
That means the question isn't simply: "Is it cortisol?"
It's: "What is my body trying to tell me?"
Why There Isn't One Simple Explanation
One of the reasons sleep can feel so confusing is because it isn't controlled by one hormone.
It's regulated by an extraordinary network involving your brain, circadian rhythm, nervous system, reproductive hormones, metabolism, and stress response. Each of these systems constantly communicates with the others.
Think of them as musicians in an orchestra. When one instrument falls slightly out of rhythm, the entire performance changes. That's why two women can experience exactly the same symptom, waking every night around 3 AM, yet have completely different underlying reasons.
For one woman, fluctuating hormones during perimenopause may be the main driver. For another, chronic stress and an overactive nervous system play a much bigger role.Someone else may have developed sleep apnea, be living with an overactive thyroid, or simply notice that alcohol consistently disrupts their sleep later in the night.
Rather than searching for one universal explanation, I believe it's far more helpful to understand your pattern. Because that's where lasting change begins.

Is It Really Cortisol?
One of the first things many women wonder is whether cortisol is to blame. It's a logical question. Cortisol is often referred to as the stress hormone, but that's only part of its story.
Produced by the adrenal glands, cortisol helps regulate your sleep–wake cycle, blood sugar, blood pressure, immune function, inflammation, and the way your body responds to physical and emotional challenges.
Under healthy circumstances, cortisol follows a beautifully predictable daily rhythm.
Levels are naturally lowest during the first half of the night, allowing for deep, restorative sleep. Towards the early morning, cortisol gradually begins to rise, helping your body prepare to wake naturally.
This daily rhythm is known as your circadian rhythm, and it's one of the reasons morning sunlight, regular meal timing, and consistent sleep habits have such a big impact on how rested we feel.
But our biology is rarely as simple as high cortisol equals poor sleep. Modern sleep medicine now recognises insomnia as a multifactorial condition. According to the 2023 European Insomnia Guideline, disrupted sleep develops through the interaction of biological, psychological, and behavioural factors, not because of one hormone acting alone.
Chronic stress can certainly influence cortisol regulation. Poor sleep can also affect cortisol. And sometimes both are being driven by something entirely different, such as fluctuating reproductive hormones, an overactive nervous system, chronic pain, or another underlying medical condition.
That's why I'm always cautious when I hear someone claim they've found the reason you're waking at 3 AM. Our bodies rarely work that simply.
The Role of Stress and Your Nervous System
One of the biggest misconceptions about sleep is that it starts when your head touches the pillow. In reality, good sleep begins long before bedtime.
Your nervous system is constantly deciding whether your body feels safe enough to rest.
When life has been particularly demanding, balancing work, caring for children, navigating relationship challenges, worrying about fertility, or simply carrying the invisible mental load that so many women experience, your nervous system can remain in a more alert state than you realise.
This doesn't necessarily mean you feel anxious.
Many women tell me: "I don't feel stressed."
And yet, when we look more closely, they're juggling countless responsibilities while rarely allowing themselves moments of true rest. Over time, this state of chronic vigilance can make it harder for the brain to stay asleep, even when you're physically exhausted.
Sleep researchers sometimes describe this as hyperarousal, a state in which the brain remains more alert than it should during the night. It's considered one of the key mechanisms involved in chronic insomnia.
This is also why improving sleep isn't always about finding the perfect supplement. Sometimes it's about gently teaching your nervous system that it's safe to slow down again.
Perimenopause: A Common Reason Sleep Changes
If you're in your late thirties or forties and your sleep has suddenly changed, perimenopause may be part of the picture. Many women expect menopause to begin with hot flushes or irregular periods, but for some, disrupted sleep is one of the very first signs.
Perimenopause is the transitional phase leading up to menopause. During this time, your ovaries don't simply produce less estrogen and progesterone, they produce them less predictably. Hormone levels fluctuate from month to month, and sometimes even from day to day.These fluctuations can affect several systems involved in sleep.
Estrogen plays an important role in regulating body temperature, supporting serotonin production, and influencing melatonin, the hormone that helps regulate your sleep-wake cycle. Progesterone, on the other hand, has naturally calming properties and supports relaxation. As progesterone production becomes more irregular after ovulation, many women notice they become lighter sleepers or wake more easily during the second half of their cycle. Some women experience obvious night sweats. Others never have them. Instead, they simply find themselves wide awake at 3 AM, wondering why sleep suddenly feels so different, than it did a few years ago.
Sleep disturbances are recognised as one of the most common symptoms during the menopausal transition and deserve to be taken seriously rather than dismissed as "just getting older." (NICE Menopause Guideline, 2024).
Could It Be Your Blood Sugar?
Another possible piece of the puzzle is blood sugar regulation.Even while you're asleep, your brain requires a continuous supply of glucose.
If blood glucose falls overnight, your body responds by releasing what are known as counter-regulatory hormones, including glucagon, adrenaline and cortisol, to restore blood sugar to a healthy range. These hormones are incredibly important, but they also increase alertness, which may contribute to waking during the night in some people. (Cryer, 2016; American Diabetes Association, 2024).
This doesn't mean that every woman waking at 3 AM has unstable blood sugar.Far from it. But if you regularly skip dinner, eat very little protein throughout the day, drink alcohol in the evening, or wake feeling hungry, shaky or with a racing heart, it may be one factor worth exploring.
Rather than aiming for "perfect" blood sugar, I prefer focusing on something much simpler: building balanced meals that provide steady energy throughout the day.
Including enough protein, healthy fats, and fibre can help support more stable glucose levels while also keeping you fuller for longer.
Other Factors That Can Influence Your Sleep
Sleep is influenced by far more than hormones alone. Several everyday habits can quietly affect the quality of your sleep without you even realising it.
Alcohol
Although a glass of wine may help you feel sleepy, alcohol actually disrupts the second half of the night. Research consistently shows that it reduces sleep quality, increases night-time awakenings and suppresses REM sleep, leaving many people feeling less rested the next morning. (American Academy of Sleep Medicine; European Insomnia Guideline, 2023).
Caffeine
Some women metabolise caffeine more slowly than others. Even an afternoon coffee may still be circulating in the body by bedtime, particularly during perimenopause when hormonal changes can alter the way caffeine is processed. (European Food Safety Authority; Baker & de Zambotti, 2023).
Body Temperature
Your core body temperature naturally drops before sleep. Anything that interferes with this process, such as a warm bedroom, heavy bedding or hot flushes, can make it harder to stay asleep. Keeping your bedroom cool, wearing breathable fabrics, and avoiding heavy meals close to bedtime can make a surprising difference. (The Menopause Society, 2023).
Sleep Apnea
Although often associated with men, sleep apnea also affects women, particularly after menopause. Loud snoring, gasping for air during sleep, morning headaches, or persistent daytime fatigue are all signs worth discussing with your doctor. (American Academy of Sleep Medicine, 2021).
What Actually Helps?
One of the biggest myths in the wellness world is that there's one supplement or one hormone responsible for fixing sleep.The research tells a different story.
The most effective approach is usually a combination of small, consistent habits that support your body's natural rhythms. (European Insomnia Guideline, 2023).
Here are a few places I often encourage women to start:
Get outside into natural daylight within the first hour after waking. Morning light helps regulate your circadian rhythm and supports healthy melatonin production later that evening.
Eat enough throughout the day. Skipping meals or chronically under-eating places additional stress on the body.
Include protein, healthy fats,and fibre with your meals to support steady energy and blood sugar.
Reduce caffeine later in the day if you notice you're particularly sensitive to it.
Create a calming evening routine that signals safety to your nervous system. Gentle stretching, reading, breathing exercises or simply dimming the lights can all help.
Move your body regularly. Exercise is one of the most effective lifestyle interventions for improving sleep quality, though vigorous exercise is generally best avoided immediately before bedtime. (World Health Organization, 2020; European Insomnia Guideline, 2023).
If sleep problems persist despite making these changes, don't simply accept them as something you have to live with.
When Should You Speak to Your Doctor?
Occasional sleepless nights are part of life. But if you're regularly waking during the night for several weeks, feel exhausted during the day, or notice symptoms such as loud snoring, gasping during sleep, significant mood changes, heavy night sweats, or unexplained weight changes, it's worth discussing this with your healthcare provider.
Sometimes disrupted sleep is linked to perimenopause. Sometimes it's related to thyroid function, iron deficiency, anxiety, sleep apnea, chronic pain or another underlying condition.
Looking at the bigger picture can help identify the most appropriate treatment rather than simply masking the symptom.

Why do I keep waking up at exactly 3 AM?
There isn't one single explanation. Waking around the same time each night may be related to your sleep cycles, stress physiology, hormonal fluctuations, blood sugar regulation, environmental factors, or an underlying sleep disorder. Looking at the bigger picture is often more helpful than searching for one isolated cause.
Can perimenopause cause early morning waking?

Is cortisol always responsible for waking at 3 AM?

Can nutrition improve my sleep?
Nutrition can certainly play a supportive role. Eating balanced meals, including enough protein, healthy fats, and fibre, limiting alcohol, and avoiding excessive caffeine later in the day may all contribute to better sleep. While food isn't a cure for insomnia, it's one important piece of supporting your overall health.
